• If your client is a person with a disability, please select the "Persons with a Disability Referral"
• Otherwise, please select the "General Referral".

If you desire to access our services at the Community Office, 561 Ellice Avenue, you can make an appointment by calling 204.925.3595 at your earliest convenience.

• If you desire to refer your client to the Community Home Services Program, please indicate that it is a CHSP referral in your communication with us.  Referrals can be made directly to ofe@ofe.ca
